Arm & Cubital Fossa

Hi!
Identify the structures in this picture. Then
a) Which muscles and skin area is supplied by B
b) What is the peculiar thing about tendon D?
c) What is the muscle supplied by E before it enters forearm?
d) what will happen to F in fracture involving medial epicondyle? what is the functional loss?
e) In supracondylar fracture of elbow, what is the after effect due to affection of G?
f) By dint of attachment to ulna by H, this muscle can cause a movement- what is the movement?
Cheers
Nilesh

  1. Hi Dr Nilesh, EK here.

    A: pectoralis minor muscle
    B: musculocutaneous nerve (c5-7)
C: short head of biceps brachii muscle
D: long head of biceps brachii muscle
    E: median nerve (c5-8, T1)
    F: ulnar nerve (c8, T1)
    G: radial artery
    H: bicipital aponeurosis

    a) musculocutaneous nv
-> biceps brachii, coracobrachialis and brachialis muscles
-> skin over the lateral part of forearm
    b) tendon of D attaches at supraglenoid fossa, which then passes through the glenohumeral capsule and exit via bicipital groove
    c) median nv supplies pronator teres before entering forearm. {it then supplies all the flexors of the forearm (except the medial 2 flexor digitorum profundus) and lateral 2 lumbricals in the hand.}
    d) ulnar nv travels under the medial epicondyle. hence, fracture of medial epicondyle may damage the ulnar nv, leading to claw hand (medial 2 fingers: extension at metacarpal pharyngeal joints and flexion at interpharyngeal joints). there are no adduction and abduction of the fingers as all the interosseous muscles are paralysed.
    e) radial artery runs of the lateral part of forearm to the hand. supracondylar fracture may damage the artery which causes the absence of radial pulse.
    f) bicipital aponeurosis allow the biceps brachii muscle to supinate the forearm

please correct me if i'm wrong.. thank you!

  2. Dear Emi
You are correct, in c) median nerve (Also except Flexor carpi ulnaris--supplied by ulnar nerve)
Also D is only intra-capsular & extra-synovial tendon running within a joint (another example popliteus in knee)
    Cheers
    Nilesh
