Interesting structures in thorax, neck, posterior mediastinum


Hi! 1. Identify the big vein R, which important vein drains blood from the thyroid into it and may be injured in 'Low Tracheostomy'?
2. Identify the nerve P, What happens when it is compressed by a metastatic lymph node or a tumour in mediastinum, How is it different from its right counterpart?
3. V is a part of aorta just distal to origin of left subclavian artery, Which congenital anomaly is common in the V-part of aorta?
4. Identify S, which parts of the body is drained by it?
5. Identify the nerve T, which important organ does it supply, what is the most important symptom in case of irritation of this nerve?
Can you tell about U-the muscle in front of which T-the nerve lies?, which important rib it is attached to? What two important vessels of the thorax it does separate on that rib?
6. Identify the vein Q, where does it drain and at what level (thoracic)?

    1. Left subclavian vein which later becomes the L brachiocephalic vein.
    2. L vagus nerve. The right vagus nerve passes anterior to the R subclavian artery but posterior to the R bronchus and SVC.
    The L vagus nerve passes in between L common carotid and L subclavian artery. the branch of the L vagus nerve - L recurrent Laryngeal nerve goes around the Aortic arch and tugs it towards the ligamentum arteriosum before it goes back cranially to innervate the muscles of the larynx. When compressed it produces hoarseness in voice.
    3. Abdominal aorta and the anomaly is a coarctation.
    4. Internal Jugular Vein - draining Brain, superficial face and neck.
    5. Phrenic nerve - diaphragm - hiccups
    The muscle is the anterior scalene. it is attached to the 1st Rib. I think it is where the axillary artery and axillary vein become the subclavian artery and vein.
    6. azygous vein t12 along with the abdominal aorta

    All answers are correct except:
    1. R is left brachiocephalic vein
    4. S is thoracic duct, it drains lymph from both side of lower part of body below the diaphragm, left half of thorax, left upper limb, left side of head & neck
    5. Phrenic nerve runs infront of Serratus anterior muscle, the muscle separates subclavian vein infront from subclavian artery behind over 1st rib
    6. Q azygos vein, drains into superior vena cava at the level of sternal angle--lower border of T-4 which is very important to delineate structures in CT scan
